FIELD TRIP PERMISSIONS AND LIABILITY WAIVER

WHO:  Limited to 20 Teens | First-come-first-served basis

DATE:  Thursday, July 11th, 2024

DESCRIPTION:  Field trip to Triple T’s Horse Ranch where they are focused on not only creating a memorable experience, but an enjoyable one. No matter your horseback riding ability, they look to create the most positive, safe, and enjoyable experience for each rider.

TRANSPORTATION:  Multi-Passenger Vans and vehicles driven by Depot-authorized drivers with clean driving records

DEPARTURE:  Meet @ Noon at The Depot (214 Main St. East Jordan)

RETURN:  Approximate return to The Depot is 3:00pm where you can pick up your teen.

COST:  FREE!!

YOU MUST RSVP:  Please submit your teen’s name in the RSVP Form and click Submit. You must also CLICK HERE to print and complete the Triple T’s Ranch waiver form.

TRANSPORTATION LIABILITY WAIVER

This form is to be completely filled out and signed by a parent or legal guardian before any child or youth under the age of 18 may be transported by Depot staff and/or volunteers for any Depot event. Permission will remain active until revoked by parent or legal guardian. If a child is dropped off or picked up for an event requiring transportation, that is considered permission for that particular event.

I hereby give my permission for my minor child to ride the shuttle van or other vehicles, including but not limited to charter buses, that are used for Depot functions. I understand that in submitting this form, I release and hold harmless The Depot Jordan Valley Teen Center and all other Depot-related functions.

By submitting this form, I release and hold harmless The Depot’s Board of Directors, employees, contractors, and any volunteers from any liability, past or future, fully and completely. I authorize the staff or designated medical professionals and/or volunteers to administer emergency medical assistance if I cannot be reached.
