IMPORTANT NOTICE BEFORE SUBMITTING

The Depot requires that each Teen who participates in activities and events is registered by a parent or guardian. The information in the form below should be provided by the parent or guardian and then updated as necessary. All information will be kept confidential.

By electronically submitting this form, you are in good faith acknowledging that BOTH the student and a parent or guardian will first review the Permissions and Agreements and then submit registration form data that is truthful and accurate.

If a Teen Registration is submitted using inaccurate information, it may result in denial or permanent loss of privileges. If it is revealed that a Teen Registration has been submitted without parent/guardian consent, it may also result in denial or permanent loss of privileges.

The Depot Jordan Valley Teen Center may contact a parent or guardian to verify that the information submitted electronically in this Teen Registration is truthful and accurate. By submitting the Teen Registration form below, you are agreeing to these terms.
