EQUINE THERAPY PERMISSIONS AND LIABILITY WAIVERS

Northern Michigan Equine Therapy requires a liability waiver form on file for each person receiving their services. Please CLICK HERE to download or print a copy to complete and submit to The Depot before the first therapy session.

The Depot JV Teen Center also requires each Teen who participates in a field trip or program that involves transportation away from a standard location to submit permission from a Parent or Guardian. (Please see the Transportation Liability Waiver information at the bottom of this page.)

OVERVIEW
Each session will include five Depot Teens, where the NMET licensed therapists provide both group and one-on-one therapy activities.

SESSION DATES
Therapy sessions are on the following 5 Tuesdays:  June 24th, July 1st, July 8th, July 22nd, and July 29th.  (It will NOT meet July 15th)

TRANSPORTATION:  Unless otherwise arranged, The Depot will pick up and drop off each teen with our passenger van at their individually assigned times. The Depot van is driven by Depot-authorized drivers who are background checked and have clean driving records.

TRANSPORTATION LIABILITY WAIVER

This form is to be completely filled out and signed by a parent or legal guardian before any child or youth under the age of 18 may be transported by Depot staff and/or volunteers for any Depot event. Permission will remain active until revoked by parent or legal guardian. If a child is dropped off or picked up for an event requiring transportation, that is considered permission for that particular event.

I hereby give my permission for my minor child to ride the shuttle van or other vehicles, including but not limited to charter buses, that are used for Depot functions. I understand that in submitting this form, I release and hold harmless The Depot Jordan Valley Teen Center and all other Depot-related functions.

By submitting the registration form, I release and hold harmless The Depot’s Board of Directors, employees, contractors, and any volunteers from any liability, past or future, fully and completely. I authorize the staff or designated medical professionals and/or volunteers to administer emergency medical assistance if I cannot be reached.
